1. With GoDaddy Hosting accounts you can grab all database information from this section in the hosting manager:
2. Once that screen opens, click this button in the database list next to your RackForms database:
3. When you click the button indicated by the arrow you will be taken to a screen similar to the one on the left hand side of the image below. For your convenience, an image of the RackForms installer is on the right with lines connecting each field:
